THE PARAMETERS THAT INFLUENCE THE EFFECTIVE FIT OF A TRANSTIBIAL PROSTHESIS: STATE-OF-THE-ART REVIEW

Abstract
Prostheses are devices that ensure the locomotion of patients who undergo the procedure of partial removal of the limb. To ensure mobility and stability for patients who have partial lower limb amputation, the load transmission between the patient's residual limb and the prosthetic socket must be optimally adjusted. The lack of a customized fit can lead to numerous health problems for the patient, as well as compromising the rehabilitation and functionality that the prosthetic process offers. In addition, the fitting of the prosthesis is one of the items responsible for the high rejection of its use by users. In this context, the article addresses a review from the identification of design parameters and relevant characteristics in the literature, in order to identify ways to reduce the impact and rejection generated by the transtibial prosthetic socket and promote a better fit of the it. The state of the art has focused on procedures, programs, materials, processes, techniques and devices. Through this study it was possible to identify how each process since the amputation to use the prosthesis is relevant to ensure a good fit and consequently re-establish the amputee's mobility. Many factors directly contribute to the patient's rehabilitation. For example, the type of amputation, the format and quality of the residual limb tissue, the type of fitting, the material and the process that is manufactured, and the sensors that monitor or assist in the identification of the intensity that the parameters are being transmitted. Furthermore, to obtain an adequate load transmission at the stump/socket interface requires that the analyses and adjustments be made individually, considering the patient's limitations and lifestyle, which makes the process costly. In this case, this article shows some current techniques and studies, such as 3D printing and advances in material technologies, that emerge as a great opportunity to enable the study of adjusted prostheses at low cost and quality. Therefore, the study presents design parameters and relevant characteristics that must be taken into account in the development of transtibial prosthetic socket, increasing the effectiveness of the adjustment and the acceptance of the prosthesis and, consequently, improving the quality of the user's life.
