CAST Analysis of an Offshore Helicopter Accident

Abstract
This study consists of an accident analysis based on the STAMP (Systems Theoretic Accident Model and Processes) methodology in order to generate safety recommendations for aviation. An offshore helicopter accident was analyzed through CAST (Causal Analysis Based on STAMP). Offshore helicopters are essential for the Oil and Gas Industry around the world. There are several hazards associated to their operations, which include: lack of situational awareness, visual spatial disorientation, and aircraft systems failures. The case studied contains some of these hazards. The safety recommendations generated by the analysis were compared to the ones presented in the Accident Investigation Report, in order to evaluate the effectiveness of the STAMP methodology. The CAST analysis produced 24 safety recommendations, against the 15 recommendations of same scope from the Investigation Report. Some of the safety recommendations produced by the CAST analysis were equivalent to the ones generated by the investigation. The results demonstrate the application of systems theory concepts for safety analysis.
